Gwinn health center recognized for excellence

GWINN, MI--   The Upper Great Lakes Family Health Center in Gwinn has received several grant awards from the U.S. Health Services and Resources Administration.

The awards recognize the highest-performing health centers nationwide and those that have made significant improvements from the previous year.

This year, Upper Great Lakes Family Health Center is getting $152,000 and being recognized for achievements in the following categories:  

  • Health Center Quality Leaders ($44,130): achieved the best overall clinical performance among ALL health centers throughout the nation and placed in the top 30% of the adjusted quartile rankings for clinical quality measures. 
  • Clinical Quality Improvers ($26,880): made at least a 10% improvement in one or more clinical quality measures between 2016 and 2017.
  • Electronic Health Record (EHR) Reporters ($5,000): employed electronic health record to report on all clinical quality measure data for all of the health center’s patients.
  • Advancing Health Information Technology (HIT) for Quality Awards ($6,000): recognized health centers that utilized health information technology systems to increase access to care and advance quality of care.
  • Achieving Patient-Centered Medical Home Recognition ($70,000)

 

Nicole was born near Detroit but has lived in the U.P. most of her life. She graduated from Marquette Senior High School and attended Michigan State and Northern Michigan Universities, graduating from NMU in 1993 with a degree in English.
